Patient FormsThe following forms are required for
your visits. When you click the link you will
bring up the .pdf version of the forms. (Please
note that this requires the Adobe reader, it is free of
charge. Click here in order to
download the latest version). You can then print these on
your printer and complete the forms. Bring them in with your for your first visit.
Please arrive 15 minutes prior to your scheduled visit to finish any additional paperwork
required, thanks.
Forms for the first visit

Patient information form with contact information:
This form contains basic
contact information like name and address. We use
this information to contact you with lab results or in
the event of an emergency. Please fill out prior
to your first visit or if there are significant updates.

Adult Data Base
/ Medical History Form: This form contains a basic medical history. Please print and fill out prior to your first visit. This makes our office run more efficiently and allows your visit to proceed quicker.

Release of
Records Form: Print this out and send to your previous doctor if you need old records sent to our office. In most cases, the doctors will not need old records.
If you choose to use this form, you may want to
check with the doctor or nurse regarding exactly
which records would be of benefit.
Other Forms

Urodynamic
questionnaire with urinary diary: Dr.
Dolnicek requires this form be completed before
urodynamic testing.

Interstitial cystitis
questionnaire: This form is the PUF
Questionnaire that the physicians have you fill out at each follow up visit for interstitial cystitis. In order to save time, you may want to complete prior to your visit.

Bowel Prep: Prior to certain surgical
procedures, your doctor may ask you to perform a
bowel prep. This bowel prep is the one
favored by our doctors.
